The third landing attempt, aborted after an in-flight explosion. Launched 11 April 1970 at 19:13 UTC with James Lovell, John 'Jack' Swigert and Fred Haise; Swigert had replaced Ken Mattingly days before launch over a rubella exposure. On 14 April at 03:08 UTC, roughly 56 hours into the flight, oxygen tank 2 in the service module ruptured, venting the command module's oxygen and power. The crew shut down the CSM and used the Lunar Module Aquarius as a lifeboat, looping around the Moon on a free-return trajectory and improvising a carbon-dioxide scrubber adapter. Splashdown 17 April 1970. The Apollo 13 Review Board traced the failure to damaged Teflon wire insulation inside the tank, after thermostatic switches never upgraded from 28 V to the 65 V ground supply welded shut during a pre-flight detanking and let heaters overheat the tank interior.
