Nir Barzilai and colleagues propose a multi-centre trial of metformin against a composite endpoint of major age-related diseases and death, explicitly designed to establish with the United States Food and Drug Administration that ageing can carry a regulatory endpoint at all. Funding has been the persistent obstacle. This record cannot state the trial's status in 2026 and does not attempt to.
