The intended first crewed Apollo flight, never flown. On 27 January 1967, during a 'plugs-out' launch rehearsal on Launch Complex 34 at Cape Kennedy Air Force Station, a fire broke out inside the command module and killed Virgil 'Gus' Grissom, Edward White II and Roger Chaffee. The Apollo 204 Review Board could not identify a single ignition source but faulted a cabin pressurised with pure oxygen above ambient pressure, extensive flammable material, vulnerable wiring and plumbing, and an inward-opening hatch that could not be released quickly. The command module was extensively redesigned — a unified quick-release hatch, non-flammable materials, protected wiring — and the mission was named Apollo 1 at the widows' request.
