Apollo 204 Review Board reports

The board delivered its findings to the NASA Administrator. No single ignition source was identified, but the report faulted the pure-oxygen cabin at above-ambient pressure, extensive flammable materials, deficient wiring and plumbing, and the hatch design. The command module was redesigned before any Apollo flew with a crew.
